Annual Physicals & Immunization Requirements

All students entering the district must have up-to-date immunizations and physical examinations (within the last 365 days before enrollment) as required by state and district regulations. If your child needs immunizations, please ensure they receive them before the start of school.

If your child is unable to get immunizations due to medical or religious reasons, please submit the appropriate documentation. You can review the immunization requirements below:

Sports Physical Requirements

Sports Physical form differs from standard physical form. To participate on the school sports team, the doctor must complete the New Jersey Pre-participation Physical Evaluation (PPE) packet. Return forms to the school nurse.

Medication Administration at School

If your child needs medication during school hours, parents/guardians must provide:

1)    Required district medical forms (each medication needs its own form)

2)    Medication in its original container with a pharmacy label

3)    Supplies to administer the medication

 

  • Asthma Medical Forms
  • Allergy / Epi-Pen Medical Forms
  • Seizure Medical Forms
  • Diabetes Care Plan (Along with the doctor’s order for the school nurse to follow)
  • Other - Medication Administration Form (each medication requires a separate form)

When to Keep Your Child Home

To prevent the spread of illnesses, please keep your child home if they exhibit any of the following symptoms:  

 

Stay Home If:

Return to school when:

Fever of 100.4°F (38°C) or higher

Fever-free for at least 24 hours without medication 

Frequent cough, difficulty breathing, or wheezing

Mild, infrequent cough or symptom-free

Vomiting or diarrhea

Symptom free, at least 24 hours

Head lice

No live lice. Proof of completing treatment must be submitted to the school nurse. 

Significant injury, strep throat, pink eye, or other contagious illness

Cleared by doctor. Bring a note to the school nurse.

Medical Absences - Notify the school nurse: 

  • Illness lasting three or more consecutive school days
  • Hospitalization or surgical recovery
  • Chronic medical conditions requiring frequent absences
  • Contagious illness requiring clearance from a physician before returning
  • Physical restrictions (such as limitations on activity due to injury)
